Primary hyperhidrosis is characterized by excessive sweating of the hands and feet, affecting about 0.6% to 1.0% of the population. Despite being a benign pathology, it can be quite disabling for daily life. There are several treatments, but surgery is the only definitive treatment. For plantar hyperhidrosis (excessive sweating of the feet), bilateral retroperitoneoscopic lumbar sympathectomy is the technique of choice.

SOCIALLY DISABLEMENT
Hyperhidrosis usually starts in adolescence and tends to improve with age. Excessive sweating is socially uncomfortable and disabling for routine activities (personal and professional). In the case of palmar hyperhidrosis (of the hands) it can make handwriting impossible because it gets on the paper and causes problems in compliance. Plantar hyperhidrosis can cause discomfort when wearing certain shoes in addition to the associated unpleasant smell.

LUMBAR SYMPATICECTOMY
Bilateral retroperitoneoscopic lumbar sympathectomy is the most effective treatment and the only one that offers long-term results. Through three small incisions in the flanks it is possible to effectively and safely reduce excess sweating in the feet. Recovery is simple and fast.
SURGERY IS THE ONLY DEFINITIVE
Periodic injection of botulinum toxin into the soles of the feet (several painful stings per session) can only get results for a few months. Bilateral retroperitoneoscopic lumbar sympathectomy offers a lasting result without the need for further treatments, which is why I prefer the technique.

TO GET GOOD RESULTS
Failure to obtain anhidrosis (absence of sweating) may result from a collateral nervous tract or cross-innervation if only the unilateral procedure is performed. With bilateral retroperitoneoscopic lumbar sympathectomy, anhidrosis usually occurs 30 minutes after the intervention. Patients are happy with the result even if compensatory hidehidrosis occurs.
Can complications occur?

Like all procedures, complications are possible. Not being a complication, compensatory sweating can be a consequence. However, this usually occurs in the treatment of palmar but not plantar hyperhidrosis.
